Information and Assistance: Case Managers are available to assist you in finding specific resources and guiding you through application processes or to assist you in other ways that enable you to access needed services. Cost: none Case Management: A Case Manager is available to assist area residents (and their families) age 60+ with a more comprehensive level of service that goes beyond Information and Assistance. Home visits may be scheduled or appointments may be made at the office. In home assessments will be provided. Information on financial benefits and supports; prescription and medical assistance, home chore and personal care assistance, housing options (including senior housing, assisted living, etc.), addressing physical and mental health as well as general wellness issues are provided. Case Managers assist the elderly person, caregiver and family. It is best to call and make an appointment. Caregiver Support Group: This group is sponsored by EMMCA and provides a forum for those who are caring for an aging loved one to seek out information and gain support from others who are in a similar situation. Each meeting is facilitated by one of our case managers. Meetings are held the second Tuesday of each month February through November from 7:00 – 8:30 in the EMMCA building. Free respite care is available for your loved one so you can attend. Nutrition: Lunch is served at Monona Hills, 353 Owen Road, Monona on Monday, Wednesday and Friday. Lunch is served at Monona Meadows, 250 Femrite Drive, Monona on Tuesday and Thursday. Both sites are open to the public but you need to call and make a meal reservation no later than noon the day prior to the day you want a meal. Transportation is available. Home Chore: Assistance with common household and yard tasks for low income seniors. The volunteer coordinator will match a volunteer with you to meet your need. All volunteers have background checks prior to being placed. Intergenerational Program: This program works with area high schools to coordinate student volunteers to assist with the home chore program by helping individual seniors and/or to provide visiting and socialization time with seniors. Adult Day, Evening and Saturday Care: Located in a homelike setting, experienced, trained staff members provide individualized programming, geared to meet the need of social, educational and physical needs and wants of participants. Our staff currently includes a music therapist and certified nursing assistant. Music, current events, arts and crafts, word games, reminiscence activities and daily gentle exercises keep participants alert and involved. Nutritious meals and snacks are provided as well. Monday through Friday services are available from 8:00 a.m. to 4:00 p.m. People may register one day a week, several days or daily. Evening care is available from 5:00 – 8:30 p.m. two Thursdays a month and from 1:00 – 4:30 p.m two Saturdays each month. Reservations must be made at least 24 hours in advance. Services are provided at Monona Meadow Apartment Clinic every other month on the fourth Friday. Call EMMCA to make reservations. Health Equipment Lending Closet: EMMCA has various equipment and supplies including canes, walkers, wheel chairs, shower benches, etc. for loan. We are also in need of donations of this type of equipment. Caregiver Resource Library: We have a wide variety of books, videos, CDs, and DVDs on many issues relating to caregiver and elderly issues.
